Abbott Reports Wednesday. The Diagnostics Moat Matters.

Abbott Laboratories reports third-quarter 2026 results before the open Wednesday, October 21. Wall Street is looking for about $1.42 in earnings per share on roughly $13.0 billion in revenue. The number the market should focus on is not EPS. It is the diagnostics segment, where analysts expect a big year-over-year jump that reflects the March 23, 2026 acquisition of Exact Sciences.

AI Meltdown has Began: Take These Five Steps Now

Most of that leap reflects the March 2026 acquisition of Exact Sciences. But beneath the M&A arithmetic sits something harder to replicate: Abbott’s position as the company that built the closest thing medicine currently has to an automated blood diagnostics lab at desktop scale.

What the Hardware Actually Does

Abbott’s Alinity platform anchors its core laboratory franchise. The Alinity ci series integrates clinical chemistry and immunoassay testing in a single footprint, and the Alinity s handles blood and plasma screening with enough throughput that Abbott says its systems and tests screen more than 50% of the world’s blood and plasma supply. Alongside it, the i-STAT system delivers blood gases, electrolytes, coagulation markers, and cardiac proteins at the patient’s bedside in minutes, not hours, off a single-use cartridge.

The patent filings are more revealing than the product brochures. Abbott has described lab system designs that target hundreds of samples per hour per square meter of instrument footprint. That is the direction: more biomarkers, smaller sample, faster cycle time, smaller machine.

The Cancer Layer Changes the Opportunity Size

The Exact Sciences deal added cancer diagnostics as a fourth distinct revenue vertical. Cologuard for colorectal screening, Cancerguard for multi-cancer early detection, Oncotype DX for breast cancer treatment planning, and Oncodetect for detecting molecular residual disease after treatment. Cancer Diagnostics grew 13.3% on a comparable basis in Q2 2026.

The Overlooked SpaceX Play

The opportunity right now is NOT to buy SpaceX – it’s to own the small, overlooked stocks that could rip triple digits as Elon expands his empire. That’s according to Tim Bohen, self-made millionaire who called Tesla at $37.

On September 30, Abbott also announced the U.S. commercial launch of SimpleScreen CRC, an FDA-approved blood-based colorectal cancer screening test developed and manufactured by Freenome and exclusively commercialized by Abbott. In the PREEMPT CRC Study, Freenome said the test detected colorectal cancer with 81.1% sensitivity. Freenome also said FDA approval means SimpleScreen CRC meets Medicare coverage criteria.

The Cancerguard test takes a different structural approach: it combines methylation signals and protein biomarkers simultaneously, because no single signal reliably identifies cancer across all types and stages. Data presented around AACR 2026 showed about 47% of positive detections were driven by methylation alone, with the remainder split between protein-only and combined signals. That multi-biomarker architecture is what makes it difficult to displace.

Where Traders Should Look in Wednesday’s Report

ABT shares have recovered sharply since July, when the company beat Q2 estimates and raised full-year guidance to $5.45-$5.60 in adjusted EPS. The stock surged roughly 12% that day. Heading into Wednesday, the consensus implies modest EPS growth year over year. Any downward revision to the cancer diagnostics acceleration guidance, or a miss in U.S. core laboratory sales on the Alinity platform, would be the pressure point.

The earnings-day reaction in diagnostics names has been asymmetric this cycle — beats get rewarded sharply, misses get punished harder. Danaher’s Q2 print showed how quickly guidance raises can reprice a diagnostics stock when the segment mix surprises to the upside.

The microfluidics market itself is expanding at a rate that gives Abbott structural tailwind. Point-of-care platforms that combine fluid handling, detection chemistry, and software in one device are commanding the majority of market value in the sector. Abbott’s installed base in hospitals, labs, and clinics means new assays effectively have a pre-built distribution network. That installed base is what competitors cannot buy quickly, regardless of their own R&D spend.

The installed-base moat argument applies across med-tech. Intuitive Surgical runs the same playbook: once a da Vinci system is in a hospital, every subsequent procedure generates recurring instrument and service revenue.

Watch the diagnostics segment margin and the Cancerguard adoption update. Those two numbers tell you more about the durability of the moat than the headline EPS will.
